How to Apply Video

Please read instructions carefully and follow them to the letter.

Four copies of your complete Youth Award Application must be mailed to the Hispanic Heritage Foundation and postmarked no later than Friday, July 6th, 2012. If you do not submit 4 copies of your complete application, you will be disqualified. Please click here to see a video on how to assemble your application.

A complete application consists of the following documents stapled in the following order.

  1. Top-Your Youth Awards Application
    Application can be printed from the homepage of your HHYA Account.
  2. Your letter of recommendation
    Place your Youth Awards ID (YAID) in the upper right hand corner of each copy. This letter can be written by a teacher, coach, advisor, mentor, employer or someone who knows the applicant outside his/her family structure. Letter must include a brief description of relationship to applicant and a statement as to how the applicant exemplifies excellence in his/her chosen award category, dedication to community service and why he or she is a role model. Letter should not exceed one page (typed please) and must include writer's name, title, address, phone number and email address. Please note that the letter of recommendation must be submitted directly by applicant with his/her complete application.
  3. Your school transcript
    Place your Youth Awards ID (YAID) in the upper right hand corner of each copy. Transcript can be official or unofficial. Transcripts MUST be included in the application package.

Remember, you must submit four copies of your application or it will be incomplete and you will be disqualified!

DEADLINE

Please mail 4 copies of your complete application in ONE envelope. All applications packages MUST BE POSTMARKED NO LATER THAN Friday, July 6th, 2012. Each envelope must be labeled with your YA ID. Applications with incomplete or missing information will not be considered.
